The challenge

Practice staff needed fast answers from the practice's own approved documents, curated by leadership, instead of searching through folders.

Everything had to run inside the practice's own Google Cloud project, on services covered by Google's HIPAA Business Associate Agreement, with access limited to the practice's own staff.

What we did

  • Milestone 1, secure foundation and access control: Google Sign-In restricted to the practice's own domain, separate staff and leadership roles, and an append-only audit log of sign-ins, questions, syncs and report views.
  • Kept the service account key in Secret Manager with no key file in the code, and added security headers and rate limiting on sign-in and chat.
  • Milestone 2, the knowledge assistant: documents that leadership curates in a Google Drive folder are synced, split and embedded, and staff get answers grounded only in those documents, with source citations.
  • Used Vertex AI with Gemini 2.5 Flash for answers and text-embedding-005 for search, deployed as a single service on Google Cloud Run.
  • Handed over an operations runbook, a leadership guide and a staff cheat sheet.

Outcome

  • Both milestones accepted, between 9 and 20 July 2026.
  • The practice accepted Google's HIPAA Business Associate Agreement in its own console, and every Google Cloud service the assistant uses is on Google's HIPAA covered services list.
  • Rated 5.0 on Upwork.
